Botulinum toxin is a neurotoxin secreted by the bacterium Clostridium botulinum.

Medically, it is mainly used for the treatment of neurological muscle disorders. In cosmetic medicine, botulinum toxin is the best and most commonly used drug to make wrinkles disappear. This is done by temporarily weakening the muscles.

There are several types of botulinum toxin, but only type A and B are used clinically. The protein botulinum toxin is also known under the name  “BTX” or “BoNT”.


How does botulinum toxin work?

Botulinum toxin is injected in tiny concentrations and inhibits the transmission of excitation from the nerve cells to the muscle, causing the muscle to stop contracting altogether, depending on the dosage.

For a muscle to contract or contract, it needs a transmission molecule called a neurotransmitter (acetylcholine), which is released by nerve endings at the junction with muscle receptors. The acetylcholine molecules, which attach to the muscle receptors, cause the contracture or shortening of the muscle cells.

When botulinum toxin is injected into the muscle, it prevents the release of acetylcholine and thus the contraction of the muscle cells.

Botulinum toxin is mainly used for the treatment and reduction of forehead and facial wrinkles. This also applies to smile lines and other fine wrinkles in the facial area.
Apart from aesthetic applications, botulinum toxin is mostly used therapeutically, especially for the treatment of migraine, excessive sweating, strabismus and bladder spasms.

Optimal results

The botulinum toxin effect works within a radius of approximately 1 cm, measured from the injection site. The number of necessary injections and the location of the injection sites depend on the course of the wrinkles as well as on the strength of the respective muscle activity.

While only a few injections are needed to weaken the lateral crow’s feet, several differently placed injections are needed in the forehead area to achieve the desired effect.

What is the procedure for botulinum toxin treatment?

Depending on the individual sensitivity to pain of the respective patient, a special anesthetic cream*) prescribed by us is applied to the facial region to be treated approx. 30-45 minutes before the actual treatment. This numbs the skin and greatly diminishes the sensation of pain.

The whole procedure usually takes no more than 15 minutes.
If hyaluronic acid is injected as a filler material for the treatment of deep wrinkles, this is done before botulinum toxin is injected into the respective muscle.
For optimal results, we recommend combining botulinum toxin with fillers.
The actual effect of fillers, such as hyaluronic acid, is usually noticeable only after 10-14 days.
The actual interruption of the neurological process at the nerve endings takes place only about 24-72 hours after the injection of botulinum toxin by your treating physician.
The final effect of botulinum toxin like filler (hyaluronic acid) usually occurs only after 10-12 days and depends on the individual patient and the product.

After botulinum toxin treatment

The use of botulinum toxin is considered a minimally invasive treatment. As a result, patients can resume their normal activities immediately afterward.

When should botulinum toxin not be used?

Botulinum toxin must not be used in cases of known hypersensitivity to any ingredient of botulinum toxin injection solution, during pregnancy, and in cases of known muscle weakness such as multiple sclerosis or myasthenia gravis.

Which side effects may occur?

Even with the most careful injections, “bruises” may occasionally occur, which are usually easily covered and disappear after a few days.
Likewise, in rare cases, there is an increased effect, which is noticeable in the forehead area with a slight lowering of the eyebrows, or a weakened effect, which requires a small follow-up correction. Very rarely, temporary headaches are also described.

How long does the botulinum toxin effect last?

The botulinum toxin effect usually lasts three to six months.

After a few months, the muscle slowly regains its original strength. Then, slowly, the original wrinkles also come back. This will then require a new treatment. With regular treatment, a beautiful, wrinkle-free skin appearance can be maintained for several years.
